Summary
On August 18 from 23:59 to 00:03 UTC (August 19), a subset of API requests (primarily AuthKit authentication endpoints) returned HTTP 500 errors. Automatic maintenance of a large, seldom-used internal index caused brief write-ahead-log contention on a primary database that exhausted connection capacity; service recovered automatically within four minutes. We identified the root cause and removed the index from production at 03:53 UTC on August 19 to prevent recurrence.
